Collaborative Family Practice Incentive Program
This program is designed to improve patient access to primary health-care providers. It provides financial incentives to family physicians who work with other family physicians and allied health-care professionals to improve patient access to their services.
 
Patient access to family physician in hospital Family physicians who have an approved plan to collaborate with other family physicians to ensure their patients have timely access to a physician and that medical services are provided to their patients when they are admitted to hospital will receive $5,000/ year, paid in biweekly instalments. (For example: two family physicians take turns covering one another’s patients in hospital, so one physician can continue to see patients in office.)
 
Working with allied health-care professionals Family physicians who have an approved plan to collaborate with other licensed health-care professionals, working in their Primary Health Care Network, will receive $5,000/year, paid in biweekly instalments.
 
ELIGIBILITY*
• Family physician (locum physicians are not eligible)
• All payment modalities
• Demonstrated, by application, specified criteria are met
 
*Eligibility is based on available funding.

The program is funded at $350,000 per year. When the fund is depleted no additional applications will be approved.

Health PEI administers this program and is solely responsible for the application process. Submit an application.
One application can be used for both incentive programs.

VALUE
Each incentive program is valued at $5,000/year. Physicians may qualify for one or both programs.
